Important Notes About This Data: This report is based on information provided to HUD by Continuums of Care in the 2021 Continuum of Care application and has not been independently verified by HUD. CoCs were instructed to collect data for a point-in-time during the last week of January 2021. The data presented in this report are limited to beds available for occupancy on the night of the count (beds under development are excluded). For inquiries about data reported by a specific Continuum of Care, please contact that jurisdiction directly. CoC contact information can be found on the HUD Exchange web site (https://www.hudexchange.info/grantees/). In some cases, a community may have listed a program in the Housing Inventory Count but did not provide sufficient information/detail for HUD to understand the number of beds/units available and the target population served. Those programs have been removed for the purposes of this report.

*HUD’s point-in-time count does not include persons or beds in Permanent Supportive Housing as currently homeless. **Other Permanent Housing (OPH) - consists of PH - Housing with Services (no disability required for entry) and PH - Housing Only, as identified in the 2020 HMIS Data Standards.
¹Family Units and Family Beds categories include units and beds for households with one adult and at least one child under age 18.

Continuum of Care (CoC) Program

The primary motive for designing the COC program is to help individuals and families who are experiencing homelessness and looking to get help to move into permanent housing. In addition to this, the program promotes community-wide planning, improves coordination, and facilitates strategic use of resources. 

In addition, this program also improves data collection and performance measurement which makes room for each community to tailor its programs to assist homeless individuals and families according to the challenges they face in their particular community.

Who is Eligible for the Continuum of Care Program?

Individuals and families, including homeless youth, who are experiencing homelessness and need assistance to move into transitional and permanent housing. The goal of the CoC program is to assist individuals and families to have long-term housing stability. 

Components of the CoC Program

There are five components for which the continuum of care program funds can be used. 

  1. Transitional Housing
  2. Permanent Housing
  3. Supportive Services
  4. Homelessness Prevention
  5. HMIS (Homeless Management Information System)

The components in the continuum of care program are in sync with the components under the emergency solution grants program. Consequently, it minimizes the administrative burden on the recipients of both programs. Its components also see that the reporting requirements and data quality benchmarks are applied to the projects.

Permanent Housing

It is community-based housing with no designated length of stay and where homeless individuals and families live as independently as possible.

The program provides funds for two kinds of permanent housing. 

  1. Permanent supportive housing for disabled people. This is rental assistance that is paired with supportive services to ensure housing stability for disabled people.
  2. Rapid Re-Housing focuses on providing relocation services and short and medium-term rental assistance to transition homeless individuals and families to permanent housing as quickly as possible 

Transitional Housing

This program is designed to provide support to homeless individuals by providing a period of up to 24 months of housing assistance and supportive services in preparation for the individuals and families to transition to permanent housing. The goal is to assist in the ability of program participants to after some time be able to self-sustain and maintain permanent housing.

Supportive Services Only

This program component is meant to provide services to homeless individuals and families who are not residing in housing owned by the recipient. The recipients are free to use the funds to conduct outreach to sheltered and unsheltered homeless persons and families such as street outreach. Providing education on resources available to the homeless population.

Homeless Management Information System

HMIS is a system used to track trends and data in homeless populations. This data is then used to measure the effectiveness of programs currently used to battle homelessness. This system is required to be used for any programs receiving federal funding.

Homelessness Prevention

Programs that are designated by HUD to assist individuals and families who are at risk of homelessness. This service provides many options in an attempt to prevent an impending situation of homelessness. Recipients and subrecipients of these allocated funds will help individuals and families who are at risk of homelessness to maintain their existing housing or transition to new permanent housing.

Other Components

Safe Havens

A Safe Haven is defined as a type of Supportive Housing Program that serves hard-to-reach homeless individuals with severe mental illness who come primarily from the streets and have been able to take part in supportive services.

The CoC will no longer fund new Safe Haven projects however prior existing Safe Haven projects will remain funded. Safe Havens that remain funded must continue to meet certain criteria such as providing drop-in services as needed.

Section 8 Moderate Rehabilitation SRO

Just like the Haven projects, the Section 8 Moderate Rehabilitation Program is also not eligible under the CoC program. However, the current SRO projects will get renewed as usual under the Multifamily Assisted Housing Reform and Affordability Act of 1997.
